Process:
- The healthcare provider submits this online application on behalf of the patient.
- The patient must complete a Release of Information (ROI) in order to qualify for financial assistance.
- The patient completes a short survey within 30 days of the financial assistance award.
- Please allow a 6-8 week turnaround cycle for funding after an application submission.
